Transitional Care program

My stroke happened 11years ago. After 3months in LGH LAunceston I went home to the Transistional Care. Wonderful care from OT physios nurses who called weekly. Social workers were so supportive while in hospital my husband is still my carer so he received such support from the team. there is a monetary cost. 

    My recovery has been amazing with help from all the allied health people beside me. .there was a time when I thought I would never walk again. My mobility is shaky ,still working on that.

Mum was operated on to fix a carotid cavernous fistula in early January of 2024. While the surgery was a success, she suffered a severe stroke as a result. Three weeks after acute care, she was moved to the GEM ward, and two weeks after that, the hospital team determined that there was little anyone could do for her, and that we should expect little improvement. Allied health services during those five weeks were ad-hoc and piecemeal. She is on the cusp of moving into a Transition Care Program, where allied health services will be focussed on maintenance, not rehabilitation.

I was told by a hospital team member that we cannot bring in outside physio services to assist Mum in her recovery because of OH&S. Is it true? And if so, what kinds of things can we, her family do  at her bedside in order to continue her rehabilitation?

I’m sorry to hear about your mum’s stroke.

If your mum’s Transitional Care Program is going to take place in a facility, then yes facilities do have rules/regulations regarding external health professionals practicing in their facility and yes this is likely due to a variety of OH&S issues that could arise.

I commend you for advocating for your mum and for trying to support further in her recovery. I would recommend speaking with her Allied Health practitioners and asking if they could develop a program for your mum that involves exercises that you and your family could do with her outside of her therapy sessions. This would enable you to continue to help and support her, but being guided by the professionals will ensure the program is working towards your mum’s goals and is manageable for your mum.

The other option would be to enquire if visiting an external physiotherapist is an option while your mum is on TCP and whether they can help facilitate this.
