The Transitional Care program helps older people (over 65) get back on their feet after a hospital stay. The program is designed to give someone more time and support after leaving hospital, so they are set up to return home and are not pressured to move into residential care before they are ready to do so. It provides short-term care for up to 12 weeks, including social work, nursing support, personal care and allied health care for maintenance (not for rehabilitation).
The package can be provided in a hospital, in a respite facility or at home. It does need an assessment by the Aged Care Assessment team to determine eligibility and this must be carried out whilst someone is still in a public hospital.
It is always best to talk to the hospital treating team about eligibility for a Transitional Care Program or Package, before you or a loved one is discharged home.
