In a post on EnableMe, I discussed the concept of “Key Moments,” which, in the context of a stroke survivor's rehab journey, are opportunities that, when embraced, can give a big boost to their momentum as they re-enter and progress in their post-stroke life recovery..
One such key moment is leaving the hospital and returning home to your family and the familiar surroundings you’ve missed since your stroke.
It can be a challenging journey, leaving a place where life has structure—a daily routine, rehab managed and monitored by skilled clinicians. In many cases, you simply followed instructions.
Now you’re heading home, where you are on your own, in a manner of speaking, and you’ll have to do your rehab with just yourself and your family for company.
In addition, there are many other issues which will emerge, which you will have to overcome, but all is not lost!

A UK Stroke Group that I am involved with is “Different Strokes UK” published a document titled Leaving Hospital After A Stroke. which I found interesting for Stroke Survivors and their families or carers. Please take the time to read it, it may help you or give some important insights.
The information contained in this document is both interesting and highly relevant for stroke survivors, as well as their families and caregivers.
