Put simply writing down objective[or if you prefer the term goals] for your rehab plan is setting out what you are planning to do to get back to “normal”.
I have been working with setting objectives all my working life and I discovered a simple system which will enable anyone who is unfamiliar with this process to compose meaningful plans to get onto the road to recovery.
It is titled the SMART process and it is as follows-
Specific- you must spell out what you are planning to achieve- avoid vague/general targets.
Measurable – You must be able to measure your target, e.g. distance walked weight loss, etc.
Attainable- Your goal must be realistic, if in doubt speak to you clinician.
Relevant- It must be linked to your recovery- otherwise it could be a waste of time.
Time-Based- There must be an end date- that give you something where you can measure progress.
