Stroke Foundation and Heart Foundation are working to develop information resources to support mental health and psychological wellbeing for survivors, families and carers. We need our stroke community to help us understand people's needs, gaps and preferences when it comes to the personal impact of stroke.

To participate in this survey, you must be aged 18 years or over and live in Australia. Participation is entirely voluntary and you can stop completing the survey at any time. The survey is confidential and anonymous. It will take about 20 minutes to complete.

If you have had a stroke, complete the survey here: https://forms.office.com/r/C2UMYx1PqA

If you support someone who has had a stroke, complete the survey here: https://forms.office.com/r/vD75AzRhmC

Hi Jude.

I am a stroke and cardiac arrest survivor and still working and have completed many advanced educational courses post stroke about these very important issues and more.

I'll complete the survey for you.

Cheers Grame

Thanks Graeme, very much appreciated. This survey will help us get our new resources right for our community. So we are keen to get as many people as we can!

We are taking a broad view of psychological wellbeing and mental health. We know that people have a range of experiences, and everyone’s experience is personal to them. We believe that all  survivors, families and carers should receive information and support about these issues after a stroke.

So the survey really is for every survivor, family member and carer. If you have the time, I would be grateful if you would consider doing the survey.

Best, Jude
