1 PostsWould you like to take part in a research study that aims to improve health outcomes for Stroke survivors? Recent evidence shows us that many people are being discharged from hospital without education or care that is tailored to the individual needs of the patient. The aim of this study is to co-design and build a stroke portal that will enable patients across Metro North Hospital and Health Services (MNHHS) to access a range of personal health information on their smart phone or tablet. The app would be tailored to the health needs of the individual including individualised patient health information and post-discharge care, with links to key stroke resources.
