We’re sorry to hear about the difficulties you’ve been having accessing rehab. Depending on how long it has been since you were discharged from hospital, it is the responsibility of either the hospital or your GP. The GP is always a first option in linking you back into the rehab system but if you were only recently discharged then the hospital is responsible for making sure you have been safely discharged and set up with the services and supports you need. If there has been any change to your function or deterioration you may also benefit from a review by a neurologist (stroke doctor) or a rehabilitation physician.
We can help advise and support you with this, however we need more information to make sure our advice is appropriate and relevant. It is important to advocate for yourself until you are given the appropriate referrals.
