I think a good place to start would be with your husband’s current rehab team. Chat with them about what his options are after January. He may be able to continue therapy with the same service at a lower intensity or be involved in group exercise sessions. They may be able to refer him to another community therapy service that can offer further therapy to continue to monitor and progress his home exercise program as needed. You should also find out whether they have a process where your husband can be reviewed by the rehab doctor in the service in a few months’ time to check on his progress.
You can speak with your husband’s GP about being linked in with the Chronic Disease Management Plan which gives you access to up to 5 Medicare subsidised sessions with a private allied health professional per year (e.g. physiotherapy).
If your husband is under the age of 65, he can get in touch with the NDIS (National Disability Insurance Scheme). Even if the NDIS is not available in your area, they will be able to let him know who he can contact to access the right services.
