Researchers from Monash University are developing a digital tool for use by general practitioners (GPs) to monitor and support people who have experienced a stroke.
This survey will help shape the digital tool which will help GPs monitor, identify, prioritise and support the health needs of survivors of stroke. Your responses will play a vital role in ensuring that the tool can be used to collect essential information required for stroke care.
