To get up in the morning, go to the toilet, shower, dry and dress by myself.

Time Frame:

Best Outcome:

I will get my independence back and not have to rely on people. This will feel marvellous.

Steps and Action Plan:

• Step 1:

  Get out of bed on my own

  • Activity 1:

    Practise getting in and out of bed

• Step 2:

  Walk to the toilet as independently as possible

  • Activity 1:

    Use 4ww and stand by assist

• Step 3:

  Adjust clothing and toilet independently

  • Activity 1:

    Use over toilet aid

• Step 4:

  Get into shower, use taps, wash body

  • Activity 1:

    Use shower chair

• Step 5:

  Dry upper and lower body then dress upper and lower body while seated

  • Activity 1:

    Seated in shower chair, use aids as needed
